For Dependent and 100% DAV card issuance, renewal, and replacement
Step 1
Request a document Drop-Off Link
- All non-military email affiliated applicants must email 60FSS.FSPS.DEERS-IDCARDS@us.af.mil to request access to the DOD Safe drop-off website.
Step 2
Await an email notification for DOD Safe document drop-off authorization.
- We will send you a link to the DOD Safe website where you will upload your documents. This email will be sent within 2 business days.
Step 3
Sign and save the forms below
- MilConnect DD1172-2
- e-ID Card Renewal Form
- Manual DD1172-2
- Take a photo against a white background.
- Review photo guidelines or download the Passport Photobooth App (IOS or Android)
- Photos must be 5×7 inches
